Our E-forms are designed to save you time and effort.  I know that you can go to MS Word or Excel and whip up your own health form.  It’s really not hard, but why go to all that trouble when we have already done that for you.

You may say to yourself that you have no need for any home health care forms, but they really do help you provide the best care for your parents.

If you take your elder’s blood pressure daily you can easily see if they need to have a change in their medication and your physician can readily see if an increase or decrease is needed.  This eliminates the guessing by you and your doctor.

We have spent many hours figuring out what information  really needed to be on the individual healt history form papers.  We have gone back and done many revisions to them because our first or second try didn’t quite get all that was needed or it had info we didn’t need.

Dad’s physician would look at the health chart or forms and she would also make suggestions on what would be helpful to her caring for him.
